What is the Relative Fat Mass (RFM) Index?

The Relative Fat Mass (RFM) equation is a validated, simple formula for estimating whole-body fat percentage using only two basic anthropometric measurements: standing height and waist circumference. Developed in 2018 by researchers Orison O. Woolcott, MD, and Richard N. Bergman, PhD, at the Cedars-Sinai Diabetes and Obesity Research Institute (published in Scientific Reports / Nature Publishing Group), the RFM metric was calibrated and validated against dual-energy X-ray absorptiometry (DXA) whole-body scans across more than 12,000 diverse participants in the National Health and Nutrition Examination Survey (NHANES).

The Relative Fat Mass Formulas

Unlike traditional body mass index (BMI) which relies solely on total body mass and cannot differentiate between dense skeletal muscle and adiposity, RFM utilizes the ratio of height to waist circumference:

Adult Formulas (Ages 15 and Older)

Note: Both height and waist circumference must be expressed in the same units (e.g., centimeters or inches).

Pediatric Formulas (Ages 8 to 14 Years)

In 2019, Woolcott and Bergman expanded the model to children and adolescents (published in Pediatric Obesity):

According to the American Council on Exercise (ACE) and clinical guidelines, body fat percentage estimates fall into the following ranges:

Why RFM Outperforms Traditional BMI

While BMI ($kg/m^2$) is widely used due to convenience, it frequently misclassifies athletic individuals with dense muscle mass as overweight, while missing normal-weight individuals with high central visceral adiposity (sometimes called "skinny fat" or metabolically obese normal weight). Because the waist circumference directly captures visceral intra-abdominal adipose tissue, RFM demonstrates significantly higher diagnostic accuracy, fewer false positives, and fewer false negatives when benchmarked against clinical DXA imaging.

How to Correctly Measure Your Waist

To ensure accurate calculations:

  • Stand upright with feet shoulder-width apart and relax your abdomen.
  • Place an inelastic measuring tape horizontally around your waist at the top of your hip bones (the iliac crest), roughly at the level of the navel.
  • Ensure the tape is snug against the skin without compressing soft tissue.
  • Take the measurement at the end of a normal, relaxed exhalation.

Why is body weight optional in this calculator?

What is the waist-to-height ratio (WHtR) and why is it important?

The waist-to-height ratio evaluates central fat distribution. A ratio under 0.5 (meaning your waist circumference is less than half your height) is widely recognized as a healthy threshold associated with lower risks of type 2 diabetes, cardiovascular disease, and hypertension.

Can pregnant women use the RFM formula?

No. Pregnancy expands abdominal circumference due to the developing fetus, amniotic fluid, and uterine enlargement, making waist-based fat percentage estimates inaccurate during gestation and early postpartum.

Does RFM work for children under 8 years old?

The pediatric RFM equation was derived and validated for children aged 8 to 14 years. It is not recommended for children under 8 years old due to distinct developmental body proportions.
