What Is Absolute Lymphocyte Count?

Absolute lymphocyte count (ALC) is the total number of lymphocytes per microliter of blood, calculated from the white blood cell (WBC) count and the lymphocyte percentage on a complete blood count (CBC). ALC reflects overall lymphocyte mass and is used in immunology, oncology, and HIV care. Unlike relative lymphocyte percentage alone, ALC accounts for changes in total WBC and gives a more reliable measure of lymphocyte status.

How ALC Is Calculated

The formula is: ALC = WBC × (lymphocyte % ÷ 100), with WBC expressed in × 10³ cells/µL (thousands per microliter). For example, a WBC of 7.0 with 30% lymphocytes yields an ALC of 2.1 × 10³ cells/µL. Both values come from the same CBC differential and should be interpreted together with clinical history and other laboratory findings.

ALC and HIV CD4 Screening

In resource-limited settings, ALC can help identify patients who may have CD4 counts below 200 cells/µL, a threshold associated with AIDS and increased opportunistic infection risk. An ALC below 0.95 × 10³ cells/µL suggests a high likelihood of CD4 below 200. An ALC above 1.7 makes CD4 below 200 less likely, though direct CD4 testing remains the standard for HIV staging and treatment decisions.
