What Is the Warsaw Method in Diabetes Management?

The Warsaw Method (also known as the Pańkowska method) is an advanced insulin dosing strategy developed for individuals with Type 1 diabetes who use continuous subcutaneous insulin infusion (CSII / insulin pumps). While traditional carb counting calculates insulin boluses solely based on dietary carbohydrates, meals rich in fats and proteins (such as pizza, steak, lasagna, and barbecue) cause delayed postprandial hyperglycemia that peaks 3 to 8 hours after eating.

The Warsaw Method translates the energetic content of fats and dietary proteins into Fat-Protein Units (FPU) to calculate both the additional insulin dose and the optimal duration for a dual-wave (combo) or extended square-wave bolus. How the Warsaw Method Calculates FPU and Extended Boluses

The method uses standard nutritional energy conversions:

  • Fat Energy (kcal) = Fat (grams) × 9 kcal/g
  • Protein Energy (kcal) = Protein (grams) × 4 kcal/g
  • Total Fat-Protein Calories = Fat Energy + Protein Energy
  • Fat-Protein Units (FPU) = Total Fat-Protein Calories / 100
  • Carbohydrate Equivalent = Total Fat-Protein Calories / 10 = FPU × 10 grams

Dual-Wave Bolus Splitting and Extended Duration

With an insulin pump, the total meal bolus is split into:

  1. Normal Bolus (Immediate): Covers the fast-acting carbohydrates: Normal Insulin (U) = Carbs (g) / ICR.
  2. Extended Bolus (Square Wave): Covers the delayed fat and protein absorption: Extended Insulin (U) = Carb Equivalent (g) / ICR.

Frequently Asked Questions

Why do fats and proteins require insulin?

Fats slow gastric emptying, delaying glucose absorption from carbohydrates, while dietary amino acids stimulate hepatic gluconeogenesis and glucagon release. Without an extended bolus, high fat-protein meals often cause early hypoglycemia followed by stubborn late hyperglycemia 4 to 8 hours later.

What is a dual-wave or combo bolus?

A dual-wave bolus delivers a portion of the insulin dose immediately for rapidly absorbed carbohydrates and spreads the remaining portion evenly over a set number of hours for slow-acting nutrients.

Can people on Multiple Daily Injections (MDI) use the Warsaw Method?

People using pens or syringes cannot program continuous square-wave infusions, but they can split rapid-acting injections (e.g. giving the carb dose at the meal and a second smaller dose 2 to 3 hours later) under the guidance of their endocrinologist.

What is an Insulin-to-Carbohydrate Ratio (ICR)?

The ICR represents the number of grams of carbohydrate covered by one unit of rapid-acting insulin. For example, an ICR of 10 means 1 unit of insulin is needed for every 10 grams of carbs.
