Intensive insulin therapy often requires a bolus dose before meals to cover carbohydrates and correct high blood glucose. Carbohydrate Coverage

Carbohydrate dose equals total grams of carbs divided by your carbohydrate ratio (grams covered per unit of insulin). A common starting ratio is 500 divided by total daily insulin dose, though your endocrinologist should personalize this value based on logs and response.

High Blood Glucose Correction

When pre-meal glucose exceeds your target, add a correction dose: (current glucose minus target glucose) divided by the insulin sensitivity factor (ISF). ISF is often estimated as 1800 divided by total daily insulin for rapid-acting analogs. Correction applies only when current glucose is above target.
