What Is HOMA-IR?

HOMA-IR (Homeostatic Model Assessment for Insulin Resistance) estimates insulin resistance from a single fasting blood draw. It combines fasting glucose and fasting insulin into one index. Clinicians use it alongside A1c and fasting glucose when evaluating metabolic syndrome, prediabetes, and type 2 diabetes risk. How HOMA-IR Is Calculated

The formula is (fasting insulin × fasting glucose) ÷ 405 when glucose is in mg/dL, or ÷ 22.5 when glucose is in mmol/L. Insulin must be in mU/L. Both values should come from the same fasting sample after at least 8 hours without food. This calculator also reports QUICKI, a related insulin sensitivity index derived from the same inputs.

Interpreting Your Result

Values below 1.5 often suggest normal sensitivity. Scores from 1.5 to 2.0 are borderline. Scores above 2.0 raise concern for insulin resistance, and values above 2.5 strongly suggest it. QUICKI (Quantitative Insulin Sensitivity Check Index) uses the same fasting glucose and insulin values. Lower QUICKI values indicate reduced insulin sensitivity. Values below 0.45 may suggest resistance; below 0.30 may suggest diabetes or marked resistance.

Can HOMA-IR diagnose diabetes?

No. HOMA-IR estimates insulin resistance from fasting labs. A formal diabetes diagnosis requires clinical criteria such as fasting glucose, oral glucose tolerance testing, or HbA1c interpreted by a provider.

Why do glucose units change the formula?

The original HOMA-IR equation was derived for mg/dL glucose. When glucose is entered in mmol/L, the divisor changes to 22.5 so the index stays equivalent. Insulin remains in mU/L in both cases.
