What is Corticosteroid Equivalence?
Corticosteroids are synthetic steroid hormones widely prescribed for their potent anti-inflammatory, immunosuppressive, and metabolic effects. Different steroid formulations vary significantly in their glucocorticoid (anti-inflammatory) potency, mineralocorticoid (sodium and fluid retention) activity, and biological duration of action.

How to Calculate Steroid Conversions
To convert between two corticosteroids, use the ratio of their equivalent anti-inflammatory doses:

Do all corticosteroids cause fluid retention?
No. Synthetic long-acting steroids like dexamethasone and betamethasone have virtually zero mineralocorticoid potency, meaning they cause minimal sodium and water retention compared to hydrocortisone or fludrocortisone.
Is prednisone active in the body?
Prednisone is a prodrug that must be converted by 11-beta-HSD in the liver into active prednisolone. In patients with severe hepatic dysfunction, active prednisolone or methylprednisolone is clinically preferred.
What is considered a physiologic replacement dose?
