LRINEC Score Calculator
Assess necrotizing fasciitis risk from CRP, WBC, hemoglobin, sodium, creatinine, and glucose lab values.
What Is the LRINEC Score?
The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score helps clinicians estimate the probability of necrotizing soft tissue infection using six routine laboratory values: C-reactive protein (CRP), white blood cell count, hemoglobin, sodium, creatinine, and glucose.
How the LRINEC Score Is Calculated
Each parameter is assigned points based on predefined cutoffs. The total score ranges from 0 to 14. Higher scores indicate a greater likelihood of necrotizing fasciitis. Select the range that best matches each lab result to compute the score instantly.
Interpreting LRINEC Results
A score of 5 or below suggests a low probability of necrotizing fasciitis (below 50%). Scores of 6–7 indicate medium risk (50–75% probability), while scores of 8 or higher suggest high risk (above 75%). LRINEC should be interpreted alongside clinical findings, imaging, and surgical assessment — it does not replace clinical judgment.
Frequently Asked Questions
What lab values are used in the LRINEC score?
LRINEC uses CRP, WBC count, hemoglobin, sodium, creatinine, and glucose. Each value is categorized into ranges that contribute 0, 1, 2, or 4 points to the total score.
What LRINEC score indicates high risk?
An LRINEC score of 8 or higher is associated with a greater than 75% probability of necrotizing fasciitis and warrants urgent surgical evaluation.
Can a low LRINEC score rule out necrotizing fasciitis?
A low score reduces but does not eliminate the possibility. Early or atypical presentations may have normal labs. Clinical suspicion should always guide management.
When should LRINEC be used?
LRINEC is most useful when necrotizing fasciitis is in the differential diagnosis for a patient with soft tissue infection and laboratory results are available.
What is the maximum LRINEC score?
The maximum possible LRINEC score is 14, obtained when all six parameters fall in their highest-risk categories.
