DLBCL Prognosis Calculator
Estimate diffuse large B-cell lymphoma survival with the revised International Prognostic Index score.
What Is the Revised IPI for DLBCL?
The revised International Prognostic Index (R-IPI) stratifies prognosis in diffuse large B-cell lymphoma (DLBCL) treated with rituximab plus chemotherapy (R-CHOP era). Five factors each add one point: age greater than 60 years, Ann Arbor stage III or IV, elevated serum LDH, ECOG performance status 2 to 4, and more than one extranodal site of disease. The total score from 0 to 5 maps to four-year progression-free and overall survival estimates.
Four-Year Survival by Score
Score 0: about 94% four-year PFS and OS (very good prognosis). Score 1: roughly 80% PFS and 79% OS. Score 2: about 65% PFS and 67% OS. Score 3: approximately 51% PFS and 55% OS (intermediate). Scores 4 to 5: about 38% to 21% four-year survival (poor prognosis). These figures derive from R-CHOP cohort validation and support treatment intensity discussions.
Clinical Use in DLBCL
R-IPI helps oncologists communicate expected outcomes and consider clinical trial enrollment or intensified therapy for high-risk patients. It does not replace histologic review, molecular subtype testing (such as cell-of-origin), or PET response assessment. Low scores support standard R-CHOP while high scores may prompt alternative regimens.
Frequently Asked Questions
What age threshold adds a point in R-IPI?
Age greater than 60 years at diagnosis adds one point to the revised IPI score.
Which stages count as advanced disease?
Ann Arbor stage III or IV disease adds one point, indicating spread beyond a single nodal region or extranodal site with regional involvement.
What four-year survival does score 0 predict?
R-IPI score 0 is associated with about 94% four-year progression-free survival and 94% overall survival in classic R-CHOP cohort data.
Does elevated LDH affect the score?
Yes. Serum lactate dehydrogenase above the laboratory upper limit of normal adds one point.
How is R-IPI different from the original IPI?
R-IPI was validated in the rituximab era and groups scores into clearer risk bands with updated four-year survival estimates for DLBCL treated with R-CHOP.
