What Is the Duke Activity Status Index?

The Duke Activity Status Index (DASI) estimates functional capacity from twelve yes-or-no questions about daily activities. Each activity carries a metabolic equivalent (MET) weight. The total score converts to estimated peak oxygen uptake (VO₂) and METs, which clinicians use for cardiovascular risk stratification before surgery. How the Score Is Calculated

Add the MET weights for every activity the patient can perform without chest pain, significant dyspnea, or undue fatigue. Estimated VO₂ peak equals 0.43 times the DASI score plus 9.6 mL/kg/min. METs equal VO₂ divided by 3.5. A DASI score of 34 or below flags elevated perioperative cardiovascular risk in validation studies.

Clinical Use

DASI is a self-report measure suitable for office visits and preoperative clinics. It does not replace exercise testing when high-risk surgery is planned, but it helps identify patients who may need further cardiopulmonary evaluation.

Does DASI replace a stress test?

No. DASI estimates capacity from self-reported activity. Formal exercise or pharmacologic stress testing may still be required for high-risk procedures.
