Understanding Peak Expiratory Flow Rate (PEFR)

Peak Expiratory Flow (PEF or PEFR) measures the maximum speed of air in liters per minute (L/min) that a person can forcefully exhale after taking a full, deep inspiration. Peak flow monitoring is a cornerstone of objective respiratory assessment for individuals living with asthma, chronic obstructive pulmonary disease (COPD), and exercise-induced bronchospasm.

How Predicted Peak Flow Is Calculated

Normal lung volumes vary naturally with biological sex, standing height, and age. Reference values are established from population regression equations:

Adult Men (EU/EN 13826 Standard):

Asthma Action Plan Color Zones

A peak flow meter acts as an early warning detection system for airway inflammation and narrowing before overt wheezing or shortness of breath becomes noticeable. Standard asthma management categorizes readings into three functional zones:

  • Green Zone (80% to 100% of baseline): Good control. Airway obstruction is minimal or absent. Symptoms are well managed. Individuals should continue their usual daily long-term control medications.
  • Yellow Zone (50% to 79% of baseline): Caution and impending flare-up. Indicates reversible airway narrowing. May coincide with nocturnal awakenings, cough, or exercise intolerance. Follow physician guidance to take prescribed short-acting beta-agonists (SABA rescue inhaler) or step up maintenance controller therapy.
  • Red Zone (Below 50% of baseline): Medical emergency. Severe broncho-constriction with imminent respiratory compromise. Take immediate fast-acting bronchodilator rescue medication and seek emergency medical evaluation without delay.

Personal Best vs. Predicted Values

While statistical population averages provide a useful starting benchmark, asthma specialists prioritize an individual's personal best peak flow. A personal best is determined by recording daily peak flow readings over a two- to three-week period of stable, symptom-free asthma control.

How do I take a correct peak flow reading?

Stand up tall, ensure the meter slider is at zero, take the deepest breath possible, place your lips tightly around the mouthpiece without blocking the opening with your tongue or teeth, and blow out as hard and as fast as you can in a single sharp burst. Repeat 3 times and record the highest of the three attempts.

What is the difference between EU and Wright scales?

Older peak flow meters used the Wright-McKerrow scale, which overestimated flow rates in the mid-range. Modern meters manufactured after 2004 comply with the EN 13826 European standard (EU scale), providing a linear and more accurate airflow calibration across all lung volumes.

When during the day should peak flow be checked?

Pulmonary function demonstrates circadian diurnal variability, typically lowest in the early morning and highest in the late afternoon. For routine tracking, measure peak flow twice daily: upon waking before using bronchodilators, and again in the evening.

Can peak flow replace formal spirometry?

No. Peak flow meters measure only the initial transient peak velocity of exhalation (PEF). Formal diagnostic spirometry measures complete forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC), and full flow-volume loops necessary for diagnosing restrictive versus obstructive lung diseases.
