The Pearl Index, introduced in 1933 by American biologist and biostatistician Raymond Pearl, remains the standard epidemiological and clinical metric for evaluating the reliability and effectiveness of birth control and family planning methods. It defines the number of unintended pregnancies that occur among 100 women using a specific contraceptive method over one full year of exposure (100 woman-years).
Mathematical Formula for the Pearl Index

Understanding the Values: Lower Is Better
A lower Pearl Index represents higher contraceptive efficacy and fewer accidental pregnancies:
- Tier 1: Most Effective (< 1% failure rate): Long-Acting Reversible Contraceptives (LARC) such as subdermal contraceptive implants (0.05), vasectomy (0.15), hormonal IUDs (0.2), and copper IUDs (0.8).
- Tier 2: Moderately Effective (1% to 9% failure rate): Injectable progestins (Depo-Provera), combined oral contraceptive pills, contraceptive patches, vaginal rings, and male condoms with perfect use.
- Tier 3: User-Dependent / Typical Use (10% to 25% failure rate): Male condoms under typical real-world use (13%), female condoms (21%), withdrawal / coitus interruptus (22%), and calendar/fertility awareness rhythm methods (24%).
- Tier 4: Least Effective (> 25% failure rate): Spermicides used alone (28%) or unprotected intercourse without contraception (~85%).
