What Is the MAGGIC Heart Failure Score?

The MAGGIC (Meta-Analysis Global Group in Chronic Heart Failure) score estimates mortality risk and survival in ambulatory heart failure patients. It combines age, sex, BMI, creatinine, ejection fraction, systolic blood pressure, NYHA class, medication use, and comorbidities into a 0–50 point score with linked 1-year and 3-year survival estimates. Inputs and BMI Calculation

Enter height in centimeters and weight in kilograms; BMI is calculated automatically. Creatinine should be in µmol/L. Ejection fraction (EF) and systolic blood pressure (SBP) come from echocardiography and vital signs. Checkboxes capture ACE inhibitor/ARB use, beta blocker use, heart failure duration ≥ 18 months, COPD, smoking, and diabetes.

Interpreting Survival Estimates

Higher MAGGIC scores indicate worse prognosis. The calculator maps the score to population-derived 1-year and 3-year survival percentages. Individual outcomes vary with treatment optimization, device therapy, and comorbidity control. Use results for counseling context, not as definitive predictions.

Scores range from 0 to 50, with higher values indicating higher predicted mortality. The score is capped at 50 in this model.

Which creatinine unit is required?

Enter serum creatinine in µmol/L (micromoles per liter), as used in the original MAGGIC meta-analysis cohorts.

How is BMI used in the score?

BMI is computed from height and weight. Both underweight and overweight ranges contribute points differently in the MAGGIC algorithm.

Does taking ACE inhibitors affect the score?

Patients not on an ACE inhibitor or ARB receive one additional point, reflecting worse outcomes in untreated cohorts.

What NYHA classes are supported?

Select NYHA Class I through IV. Class II adds 2 points, Class III adds 6, and Class IV adds 8 to the total MAGGIC score.
