What Is the GAD-7?

The Generalized Anxiety Disorder 7-item scale (GAD-7) is a brief questionnaire that screens for anxiety severity over the past two weeks. It asks how often the patient was bothered by seven core anxiety symptoms. Each item scores 0 to 3 by frequency, for a total range of 0 to 21.

Scoring and Severity Bands

Scores of 0 to 4 suggest minimal anxiety. Scores of 5 to 9 indicate mild anxiety. Scores of 10 to 14 suggest moderate anxiety. Scores of 15 to 21 suggest severe anxiety. A score of 10 or higher often warrants further clinical evaluation. Frequency Response Options

Not at all scores 0, several days scores 1, more than half the days scores 2, and nearly every day scores 3. All seven items must be answered to compute a valid total score.
