What Is the Braden Scale?

Scoring

Five subscales score 1 to 4 points. Friction and shear scores 1 to 3. Total score ranges from 6 to 23. Lower scores mean higher risk.

  • 18 to 23: average risk
  • 16 to 17: mild risk
  • 13 to 15: moderate risk
  • 12 or less: high risk

Clinical Use

Braden scores support nursing assessment but do not replace clinical judgment. Institutions may set their own cutoffs based on local validation studies.
