What Is the BODE Index?

The BODE index predicts mortality and disease severity in chronic obstructive pulmonary disease (COPD). It combines body mass index (B), airflow obstruction (FEV1), dyspnea (MMRC), and exercise capacity (6-minute walk distance). Scores range from 0 to 10, with higher scores indicating worse prognosis. BODE Scoring Components

FEV1 (% predicted): ≥ 65 = 0, 50–64 = 1, 36–49 = 2, ≤ 35 = 3. Six-minute walk distance: ≥ 350 m = 0, 250–349 = 1, 150–249 = 2, ≤ 149 = 3. BMI: > 21 = 0, ≤ 21 = 1. MMRC dyspnea: 0–1 = 0, 2 = 1, 3 = 2, 4 = 3.

Survival Interpretation

Score 0–2: approximately 80% 4-year survival (low risk). Score 3–4: ~67% (moderate risk). Score 5–6: ~57% (high risk). Score 7–10: ~18% (very high risk). The BODE index outperforms FEV1 alone for predicting outcomes in COPD.

What is a good BODE index score?

Lower scores indicate better prognosis. A score of 0–2 is considered low risk with approximately 80% 4-year survival. Scores above 6 indicate very high risk.

What is the MMRC dyspnea scale?

The Modified Medical Research Council (MMRC) scale grades breathlessness from 0 (only with strenuous exercise) to 4 (too breathless to leave home). It is a standard COPD assessment tool.

Why is BMI included in BODE?

Low BMI (< 21) in COPD patients is associated with worse outcomes, reflecting muscle wasting and nutritional depletion common in advanced disease.

How is the 6-minute walk distance measured?

Patients walk as far as possible in 6 minutes on a flat corridor. Distance in meters is recorded. Shorter distances indicate reduced functional capacity and worse prognosis.

Is BODE used instead of GOLD staging?

BODE complements GOLD spirometric staging by adding functional and symptomatic measures. It is particularly useful for predicting mortality beyond FEV1 alone.
