VLDL Calculator
Calculate estimated VLDL cholesterol level and VLDL/HDL ratio from blood triglycerides and total lipid profile.
What is VLDL Cholesterol?
Very Low-Density Lipoprotein (VLDL) is a major type of lipoprotein particle produced primarily by the liver to distribute endogenous triglycerides to peripheral tissues and muscle cells. Like low-density lipoprotein (LDL), VLDL particles carry apolipoprotein B-100 and contribute to the formation of atherogenic plaques in arterial walls.
Because direct measurement of VLDL requires specialized laboratory techniques such as preparative ultracentrifugation or nuclear magnetic resonance (NMR) spectroscopy, clinical lipid panels routinely estimate VLDL concentration using the established relationship between blood triglycerides and VLDL particles.
How VLDL is Calculated
In standard clinical practice, VLDL cholesterol is estimated from serum triglycerides using the Friedewald linear equation:
When measurements are in SI units (millimoles per liter, mmol/L):
For example, if fasting blood triglycerides are 150 mg/dL, estimated VLDL is \(150 / 5 = 30\text{ mg/dL}\). In SI units, 1.70 mmol/L of triglycerides yields \(1.70 / 2.2 \approx 0.77\text{ mmol/L}\) of VLDL cholesterol.
Clinical Reference Ranges & Interpretation
| VLDL Level (mg/dL) | VLDL Level (mmol/L) | Classification | Clinical Interpretation |
|---|---|---|---|
| 2 - 30 mg/dL | 0.1 - 0.8 mmol/L | Normal / Desirable | Healthy circulating level with standard cardiovascular risk. |
| > 30 mg/dL | > 0.8 mmol/L | Elevated | High triglyceride-rich remnants, increasing cardiovascular and atherogenic risk. |
| < 2 mg/dL | < 0.1 mmol/L | Low | Typically benign; seen with very low dietary fat intake or malabsorption. |
Clinical Limitations
The linear triglyceride-to-VLDL conversion is accurate when triglycerides are below 400 mg/dL (4.52 mmol/L). In patients with severe hypertriglyceridemia, chylomicrons alter the triglyceride ratio, rendering linear estimation inaccurate. In such cases, direct LDL/VLDL measurement is indicated.

Frequently Asked Questions
Why is VLDL considered bad cholesterol?
VLDL carries large amounts of triglycerides and remnant cholesterol. As it circulates and releases fatty acids, it degrades into intermediate-density lipoprotein (IDL) and small, dense LDL particles that penetrate endothelium and promote arterial plaque formation.
How does VLDL differ from LDL?
Both are atherogenic lipoproteins, but VLDL is composed primarily of triglycerides (around 50% to 70% by weight), whereas LDL is composed predominantly of cholesterol esters (around 50% cholesterol).
Can lifestyle changes reduce VLDL levels?
Yes. Reducing intake of refined carbohydrates and alcohol, engaging in regular aerobic exercise, and achieving a healthy weight are the most effective non-pharmacologic interventions to lower liver triglyceride synthesis and VLDL secretion.
Does fasting affect VLDL calculation?
Yes. Postprandial lipid panels include chylomicrons from dietary fat absorption. An 8 to 12 hour overnight fast is recommended for standard fasting lipid profile accuracy.
