What Is the SCORAD Index?

The SCORAD (SCORing Atopic Dermatitis) index is a clinically validated scoring tool created by the European Task Force on Atopic Dermatitis (ETFAD) to quantify the severity and extent of eczema. It combines three key dimensions of the disease: the extent of body surface area involved, the intensity of six hallmark clinical signs, and subjective symptoms reported by the patient.

The SCORAD Calculation Formula

The total SCORAD score is calculated using the established composite formula:

Where:

  • A (Extent): Body surface area affected by eczema (0 to 100%), estimated using the Rule of Nines. This accounts for up to 20% of the total score.
  • B (Intensity): Sum of 6 clinical signs graded from 0 (none) to 3 (severe) on a representative lesion: erythema, edema/papulation, oozing/crusts, excoriation, lichenification, and dryness (max sum = 18). This accounts for up to 60% of the score.
  • C (Subjective Symptoms): Sum of Visual Analogue Scale (VAS) scores (0 to 10 each) for pruritus (itch) and insomnia (sleep loss) over the last 3 days (max sum = 20). This accounts for up to 20% of the score.

SCORAD Severity Classification

The total SCORAD score ranges from 0 to 103:

  • Mild Atopic Dermatitis: Score < 25. Typically managed with barrier restoration emollients and mild topical corticosteroids or calcineurin inhibitors.
  • Moderate Atopic Dermatitis: Score between 25 and 50. Indicates active flare-ups requiring stepped-up topical anti-inflammatory treatments and structured skin care regimens.
  • Severe Atopic Dermatitis: Score > 50. Often necessitates specialist referral, systemic immunomodulators (biologics, JAK inhibitors), or targeted phototherapy.

Rule of Nines for Extent Estimation

Extent estimation uses the standardized anatomical proportions:

  • Head & Neck: 9%
  • Upper Limbs: 18% (9% per arm)
  • Anterior Trunk (Chest & Abdomen): 18%
  • Back & Buttocks: 18%
  • Lower Limbs: 36% (18% per leg)
  • Genitals: 1%
