The Serum-Ascites Albumin Gradient (SAAG) is a diagnostic metric used by clinicians to classify the underlying cause of peritoneal fluid accumulation (ascites). Developed to replace the older, less accurate concept of transudates versus exudates, SAAG directly reflects hydrostatic pressure differences in the portal microcirculation with approximately 97% diagnostic accuracy.

SAAG Calculation Formula

SAAG is calculated by subtracting the albumin concentration of the ascitic fluid from the serum albumin concentration measured on the same day:

Clinical Classification and Differential Diagnosis

1. High SAAG ($\ge 1.1\text{ g/dL}$ or $\ge 11\text{ g/L}$) — Portal Hypertension Present

A high gradient indicates elevated hydrostatic pressure within the hepatic sinusoids or portal venous system:

  • Low Ascites Protein (< 2.5 g/dL): Cirrhosis (alcoholic or viral hepatitis, NASH), massive hepatic metastases, late-stage Budd-Chiari syndrome.
  • High Ascites Protein ($\ge 2.5\text{ g/dL}$): Cardiac ascites (congestive heart failure, severe tricuspid regurgitation), constrictive pericarditis, early Budd-Chiari syndrome, sinusoidal obstruction syndrome.

2. Low SAAG ($< 1.1\text{ g/dL}$ or $< 11\text{ g/L}$) — Non-Portal Hypertension

A low gradient indicates that portal hypertension is absent. The fluid accumulation is driven by peritoneal inflammation, malignancy, or severe systemic hypoproteinemia:

  • High Ascites Protein ($\ge 2.5\text{ g/dL}$): Peritoneal carcinomatosis (ovarian, gastric, or colon adenocarcinoma), peritoneal tuberculosis, fungal peritonitis, pancreatic ascites (pancreatitis or duct disruption), biliary ascites, serositis in systemic lupus erythematosus.
  • Low Ascites Protein (< 2.5 g/dL): Nephrotic syndrome, severe protein-losing enteropathy, extreme malnutrition.

Diagnostic Accuracy and Paracentesis Guidance

To ensure reliable results:

  1. Perform blood draw and diagnostic paracentesis concurrently or within a few hours of each other.
  2. Ensure samples are un-diluted and analyzed using the same laboratory reference standards.
  3. Perform diagnostic fluid cell count and differential (check PMN count $> 250/\mu\text{L}$ for spontaneous bacterial peritonitis).
