What Is Gabapentin Used For?

Gabapentin is an anticonvulsant also used for neuropathic pain, postherpetic neuralgia, and restless legs syndrome. Some clinicians use it off-label for anxiety or sleep. Dosing depends on indication, age, renal function, and titration schedule.

Common Adult Dosing Ranges

For chronic neuropathic pain, a typical starting regimen is 300 mg three times daily (900 mg/day) with titration as tolerated. Postherpetic neuralgia and adult epilepsy often begin lower and increase over several days toward 900 to 1800 mg/day in divided doses. Maximum labeled daily dose for most adult indications is 3600 mg/day. Restless leg syndrome commonly uses 600 mg once daily in the evening with food.

Pediatric Epilepsy Dosing

For children aged 3 to 12 years with epilepsy, weight-based dosing applies. Starting doses are often 10 to 15 mg/kg/day and effective maintenance ranges from 25 to 35 mg/kg/day, with younger children sometimes requiring up to 40 mg/kg/day. Daily totals are divided into three doses, with a typical maximum of 50 mg/kg/day. For general weight-based drug math, see the Dosage Calculator.

Frequently Asked Questions

Why is gabapentin titrated slowly?

Gradual titration reduces dizziness, somnolence, and ataxia. Postherpetic neuralgia and epilepsy protocols increase dose over days to improve tolerance.

How does renal impairment affect dosing?

Gabapentin is renally cleared. Reduced kidney function requires lower doses and longer dosing intervals. This calculator shows standard ranges and does not adjust for creatinine clearance.

Is gabapentin FDA-approved for anxiety or sleep?

Anxiety and sleep uses are off-label. Reported effective doses in studies vary. Any off-label use should follow prescriber guidance.

Can gabapentin be stopped abruptly?

In epilepsy, abrupt discontinuation may increase seizure risk. Taper under medical supervision, especially after long-term or high-dose therapy.

Should I use this calculator to prescribe?

This tool summarizes common reference ranges for education. Final prescribing requires review of the drug label, patient allergies, interactions, and renal function.

How many times per day is gabapentin usually given?

Most adult indications use three daily doses. Restless leg syndrome and some off-label sleep regimens use a single evening dose.
