What an Affidavit Format Is

An affidavit format is a written statement the deponent swears is true. This page fills a name-change affidavit, an address affidavit, an income affidavit, or a general affidavit, then lets you print it or save a PDF. The opening line follows the usual Indian form: name, son or daughter or wife of, age, and address, then numbered statements.

The affidavit is built in your browser. Names and the last four characters of an identity number are not uploaded. This is a format, not legal advice, and it is not notarised until you sign it before a notary or oath commissioner.

How to Fill the Affidavit Format

  1. Choose name change, address proof, income, or general. Each choice loads a filled example.
  2. Replace the deponent's name, relation, age, and address. Use Start blank if you want the headings with empty lines.
  3. Add the facts for that type. For a general affidavit, put one fact on each line.
  4. Enter only the last four characters of an identity document if you want it mentioned.
  5. Print the affidavit. Use non-judicial stamp paper of the value your state requires, then sign it before the officer who can administer the oath.

What This Format Does Not Do

It does not check a name against a gazette, calculate court fees, or tell you which stamp value your state wants. A passport office, bank, university, or court may ask for extra annexures. Read that checklist before you submit the printout. If the statement affects property, a criminal matter, or someone else's rights, ask a lawyer to draft it.

Related Formats

A no objection certificate is a different page. Use the NOC Format when an employer, landlord, or parent needs to say they do not object. Print either document on paper from the Letterhead Generator only when the authority asked for letterhead. Many affidavits belong on stamp paper instead.

Frequently Asked Questions

Which affidavit format should I pick?

Use name change when two spellings must be declared the same person. Use address proof for a present address, income for a stated yearly income, and general when you need to write your own facts.

Do I have to print the full identity number?

No. The form accepts only four characters and prints them as "ending" those characters. Leave the field blank if the affidavit should not mention an ID.

Can I start from a blank affidavit?

Yes. Start blank keeps the selected type and clears the names, so the printout still has the sworn opening and signature lines.

Is this affidavit legally valid by itself?

Not until it is signed and sworn in the way your state requires, usually on stamp paper before a notary or oath commissioner. The page only sets the wording.

Where is the affidavit saved?

Only in this browser tab until you print or download it. Closing the tab clears the form.
