- Immediate-Release (IR): Initial starting dose is typically 25 to 50 mg every 4 to 6 hours as needed for acute pain. It can be titrated by 25 to 50 mg every 3 days up to 50 to 100 mg every 4 to 6 hours. The absolute maximum daily dose is 400 mg/day.
- Extended-Release (ER): Initial starting dose for chronic pain in opioid-naive patients is 100 mg once daily. It may be titrated in increments of 100 mg every 5 days up to a maximum ceiling of 300 mg/day. ER tablets must be swallowed whole without chewing or crushing.
