What is PSA Density (PSAD)?

Prostate-Specific Antigen Density (PSAD) is a critical urological metric that adjusts total serum PSA for the volume of the prostate gland. PSA is produced by normal prostate epithelial cells as well as prostate adenocarcinoma cells. Consequently, men with large benign prostates (Benign Prostatic Hyperplasia, or BPH) often have elevated PSA levels despite having no cancer.

Role of Free PSA Percentage (% fPSA)

  • Free PSA > 25%: Associated with an 8% to 10% risk of prostate cancer (favorable).
  • Free PSA < 10%: Associated with a 45% to 56% risk of prostate cancer (high risk).
