MIPI Calculator
Calculate the Mantle Cell Lymphoma International Prognostic Index (MIPI and MIPI-b) risk score and estimated survival for patients with mantle cell lymphoma.
Understanding the Mantle Cell Lymphoma International Prognostic Index (MIPI)
Mantle cell lymphoma (MCL) is a distinct subtype of non-Hodgkin lymphoma characterized by monoclonal B-cell proliferation and cyclin D1 overexpression. Prognosis among patients with advanced mantle cell lymphoma varies widely. The Mantle Cell Lymphoma International Prognostic Index (MIPI) is the primary validated clinical risk-stratification tool developed by the European MCL Network to categorize patient risk and guide clinical management.
MIPI and Biological MIPI (MIPI-b) Mathematical Formulas
The standard simplified MIPI score incorporates four independent prognostic factors: age, Eastern Cooperative Oncology Group (ECOG) performance status, white blood cell (WBC) count, and serum lactate dehydrogenase (LDH) levels normalized to the upper limit of normal (ULN):
$$\text{MIPI} = 0.03535 \times \text{Age} + 0.6978 \text{ (if ECOG 2-4)} + 1.367 \times \log_{10}(\text{WBC}) + 0.9393 \times \log_{10}\left(\frac{\text{LDH}}{\text{ULN}}\right)$$
The Biological MIPI (MIPI-b) incorporates tumor cell proliferation by adding the Ki-67 proliferation index percentage:
$$\text{MIPI-b} = \text{MIPI} + 0.0214 \times \text{Ki-67 (\% convexity)}$$
Risk Stratification & Clinical Prognosis
Patients are stratified into three distinct risk categories based on their calculated MIPI score:
- Low Risk (Score < 5.7): Median overall survival is not reached (> 5 years), with an estimated 5-year overall survival rate of approximately 60%.
- Intermediate Risk (5.7 ≤ Score < 6.2): Median overall survival is approximately 51 months, with a 5-year overall survival rate of around 36%.
- High Risk (Score ≥ 6.2): Median overall survival is approximately 29 months, with a 5-year overall survival rate of around 26%.

Frequently Asked Questions
What is the clinical role of the MIPI score?
The MIPI score stratifies patients with advanced mantle cell lymphoma into low, intermediate, and high-risk categories. It aids oncologists in clinical trial design, risk-adapted treatment planning, and prognostic discussions.
How does Ki-67 alter the prognostic calculation in MIPI-b?
Ki-67 measures tumor cell proliferation. Higher Ki-67 values reflect more aggressive tumor kinetics. Adding Ki-67 to the standard MIPI score improves prognostic refinement, particularly for identifying high-risk biological subgroups.
Can MIPI be used for localized or relapsed mantle cell lymphoma?
MIPI was developed and validated primarily for newly diagnosed patients with stage II to IV advanced mantle cell lymphoma. While informative, its applicability in early stage I disease or relapsed/refractory settings should be interpreted alongside clinical judgment.
